Referrals can be initiated by calling our office, emailing Melody or filling out this form:

Call (905) 393-5724 or email melody@elementssupportservices.com and speak with our Director of Rehabilitation Services.

Melody Hamilton will review your current needs and make arrangements to meet with you to discuss how we can be helpful. We will likely ask you to pull together any additional relevant information for review.
